Please fill out the information below to electronically generate your proxy form for the Annual Meeting on Thursday, October 24th, 2024 at 7pm. Even if you are planning on attending the meeting, we are asking everyone to fill out a proxy form to ensure we have the required number of attendees to make an official meeting. You will retain/reclaim your vote when you arrive to the meeting in person, even if you already submitted a proxy ahead of time.
Important: Once you fill in the fields and click submit, you will be redirected to a copy of the form itself and you will be asked to digitally sign. Click on the signature area, and either draw with finger/mouse your signature, and then agree to sign. You will be emailed a pdf copy of your completed Proxy form.
