Please fill out the information below to electronically generate your proxy form for the Annual Meeting on Thursday, October 26th, 2023 at 7pm. Even if you are planning on attending the meeting, we are asking everyone to fill out a proxy form to ensure we have the required number of attendees to make an official meeting. You will retain/reclaim your vote when you arrive to the meeting in person, even if your proxy form has been filled out.
